Google says local results are decided by three things: how relevant a listing is to the search, how far it is from the person searching, and how prominent the business is. Distance you cannot change. The other two are where the work is.
For a practice, relevance means Google understands exactly which treatments you offer and where. Prominence means the rest of the web agrees you are established and trusted: consistent listings, independent mentions, and a steady flow of detailed reviews. A half-finished profile with the wrong primary category loses to a smaller practice that has done the basics properly.

Size is not a ranking factor. Usually the other practice is closer to the person searching, has a more complete profile with better categories, or has more recent and more detailed reviews. The audit shows which of those applies to you.
Within limits. Stronger relevance and prominence widen the area where you appear, and useful pages for patients traveling from nearby towns can rank in normal results. What we will not do is create addresses you do not practice from.
Google allows individual practitioner profiles, but they can compete with the practice profile for the same searches. Whether they help depends on your setup, and we decide it with you during the baseline rather than by default.
Each location gets its own profile, its own page on your site and its own listings, all consistent with the group. Reporting is split by location, so you can see which sites the map is serving well and which need attention.
